Thank you for your interest in becoming a mentor on Sports Mentorship Hub. Firstly, we'd like to find out a little about you. While filling out this application, we'd like you to share your experience and background, and tell us why you decided to become a mentor on our platform. The form only takes a few minutes to complete.

For all new mentors we require the following to be sent to us before your profile will be activated:
1. A review from a mentee you have worked with previously
2. At least one testimonial from an athlete that you have worked with previously.
3. One useful resource you have created that can be used to promote you. This must be something you are happy to share with our followers
*we will email you for the above items once we have reviewed your initial application.
